Umpqua Community Health Center would like to be your health care home. We work to help keep you and your family healthy and enjoying the quality of life you desire. Services are designed to meet the needs of people of all ages from newborn babies to senior citizens. Medical and dental services at Umpqua Community Health Clinic are provided by a team of exceptional physicians, dentists and nurse practitioners. Assisting these professionals is a team of nurses, medical and dental assistants, and hygienists as well as our front office staff.

Umpqua Community Health Center has facilities conveniently located throughout Douglas County to best serve you and your family's health care needs. Our main office is located in Roseburg at 150 Kenneth Ford Drive off of NE Stephens. We also have clinics in Glide on the North Umpqua Highway, Myrtle Creek on Main Street and in Drain on West A Avenue. In addition, we operate the Teen Health Center on the Roseburg High School campus. Our Roseburg clinic operation offers both medical and dental services.

Our services are open to anyone in Douglas County, especially those who are unemployed or work for low wages, are covered by Medicare, Medicaid or the Oregon Health Plan, or who are experiencing a barrier to health care. Umpqua Community Health Center accepts all insurances. For patients who do not have insurance, fees are charged on a sliding scale linked to their income and family size. Most uninsured patients qualify for significant discounts. Payment plans are easily arranged. We believe no one should go without necessary medical or dental care and we work to assure that, within the constraints of our budget and provider capacity, we provide as much care as possible.
